Dentrix, Eaglesoft, and Open Dental limit us, as far as what fields we can sync back.  The following article provides an overview of what fields exist in both Legwork and a PMS and whether or not we can write back to them.

2. Medical History Form
Legwork uses a static list of Medical Conditions and Allergies that write back into your PMS if they also exist inside your PMS. To ensure all of our fields connect with your software, make sure all of our conditions and allergies also exist within your PMS. 

The conditions and Allergies in our forms are below for reference:

 Where do Medical History submissions sync in my PMS?

 

Dentrix Select a Patient from within their Family Module and click the White “Health History” Cross. We write back all Medical conditions/allergies from approved forms into this section.
Eaglesoft From inside someone’s Patient File, Click Alerts. Medical conditions and Allergies will display here.
Open Dental In the Chart Module, Scroll down in the Patient Info box to see the Problems, Medications, & Allergies. Medical Conditions and Allergies will be recorded there.

Cloud9 When Medical and Dental History Forms are completed and this toggle is enabled, we write back the information as a Patient Comment.  This is accessible from Home> Comments/Alerts

 

3. Dental History Form

Dental History is currently planned to only sync with Cloud9.

4. Insurance Information Form

Insurance information is currently planned to sync with Denticon, when Insurance Information Forms are completed, we write back the Insurance Information as a Comment under Patient Notes. 

5. Authorization & Acknowledgment Forms

This form only marks patients whether or not they have agreed to the authorizations and acknowledgments terms.  It is a yes or no question within PMSs.

How do I get forms to my Patients?
All of our forms have a unique share link that can be added to any text, email or social media site. We also provide an embed code for placing on your website. 


Can you Digitize my forms?
We’ll do you one better. We have provided a full form builder to meet any form need you may have. The power of custom form creation is now in your hands!

We have also created 6 default syncing forms based upon your most commonly needed forms that are specially designed to sync with Your PMS. Feel free to add questions, unique to your office, to these forms as well! 

Can all my forms be combined into one large form?
