Introduction to Patient Forms
Legwork offers templates for patient forms. Manage, edit and customize forms with ease within the form builder. Please note that some fields cannot be edited/deleted, as they are set up to provide your practice management software with important data; however, you can add fields and re-arrange the forms according to your preference.
Navigating to Patient Forms
- Click on the Paperless Forms tab.
- Click Form Builder in the sub-navigation.
- Click on the Patient Forms folder. You can view your patient form templates here.
Editing Default Patient Forms
Please note: Fields marked with a lock symbol should not be edited or deleted. These fields are required for the form to function properly. Editing or deleting them will break the form.
- Click on a field that you would like to edit (Example: Preferred Name) A configuration pane will appear on the left side of the form builder.
- Edit the field configuration to see how the field changes
- Click the Save button in the top right corner while you continue to edit the form. Saving the form saves it "internally" within Legwork.
- Click the Update button when all the changes have been completed. Updating the form makes all recent updates visible to patients.

Creating a New Form
- Click Add a Folder.
- Create a Folder Name and click the blue Create button.
- In the top right corner click the blue New Form button.
- Create a Form Name and Select a folder to add this form to. The default Folder that was just created will appear. You can select a new folder to place this form or leave the default folder created.
- Select Sync PDF Submissions to the Patient Document Center to enable syncing.
- Using the dropdown select which folder within your practice management system this form should sync into once approved.
- Click on the blue Create Form button.
Building a Form
The Form Builder is designed to be flexible enough to build any type of form you can imagine. Whether it be a consent form for a patient or an inter-office survey, you can build it ! All forms, no matter how you build them, are mobile responsive and will look good on any device.
- Navigate to the form you want to edit and click on the form name.
- Drag and drop the fields you want to use on your form.
Tip: If you hover over the form name, you can select quick actions from the dropdown.
Creating a Multi-Column Form
- Select the Section Break field and drag it on to your form.
- Click on the Section Break you just dragged on to your form. Field configuration options will display on the left side of your screen.
- In the Section Break configuration options, click the Columns dropdown and specify if you want a 1, 2, 3, or 4 column layout.
- Drag fields into the section break field to add them to the new layout.
Adding New Pages to a Form
- Click the Add New Page link at the bottom of the form builder. This adds a new page to the form.
- Drag fields underneath the Page Break to add to the new page.

Tip: You can preview your form at any time to see how it will look with multiple pages.
